What it means
Means you feel absolutely crook, usually from a hangover, dodgy tucker, no sleep, or just a shocker of a night. It also gets used for looking filthy and wrecked, like you’ve been through the wars and shouldn’t be let out in daylight. Proper Aussie gross-out imagery that says it all in three words.
Usage examples
"Arvo after the bucks night I was rough as guts, spewin' in the dunny and lookin' like I slept in the ute."
"Woke up rough as guts after the wedding, couldn't even look at the bacon at the hotel breakfast."
"Mate, you look rough as guts, go home and sleep it off before the boss sees the state of you."
